Position Title: Consultant of Clinical Services
Project Description
The USAID-funded project, Linkages Across the Continuum of HIV Services for Key Populations Affected by HIV (LINKAGES), under the President's Emergency Plan for AIDS Relief (PEPFAR) program is implemented by FHI 360 in partnership with Pact, IntraHealth International and the University of North Carolina at Chapel Hill, and conducts a range of activities to reduce HIV transmission among key populations (KP) — sex workers, men who have sex with men, transgender persons and people who inject drugs — and improve their enrollment and retention in care and treatment across the HIV cascade. LINKAGES will accelerate the ability of governments, key population leaders, organizations working with key populations, and private-sector providers to plan and implement services that reduce HIV transmission among key populations and their sexual partners and extend the lives of those already living with HIV
Over the course of the program, LINKAGES will engage a variety of Special Experts to amplify local capacity for long-term sustainability and country ownership of key population programming. Special Experts are recognized Indonesian and expatriate TA providers who reside in Indonesia and currently work on HIV and key populations issues. These individuals will be placed on flexible consultancy agreements to help the program actualize its demand-driven TA system as well as cultivate evidence-based “push” TA efforts.
Consultant Summary / Responsibilities:
The consultant will work under the direction of the LINKAGES Technical Officer, Clinical Services and in close coordination with the LINKAGES/Indonesia clinical services team to provide senior-level expertise in the area of clinical services. He will represent LINKAGES at senior level meetings, consultations and forums, provide input to strategies, agreements, protocols, guidelines, training curricula and suggest improvements to the implementation of service delivery in government and NGO clinics. Specific activities may include:
- Provide clinical review and input during Clinical mentoring activity related to TB/HIV in DKI Jakarta targeted facilities
- Provide technical assistance on development of strategy for TB/HIV integration services at facility level
- In collaboration with Challenge TB project, support on implementation of TB/HIV integration strategy by assisting on coordination and liaison at PHO/DHO level.
- Engage MOH at national level, and especially at provincial and district levels, to develop strategy for GeneXpert access for presumptive TB among PLHIV
- Assist on development of GeneXpert protocol along with related stakeholder at National level and assist on implementation strategy on the field
- Provide guidance, input and participate in discussions in the development and finalization of TB/HIV district planning exercise and technical assistance in 5 Districts in Jakarta and 3 districts in Papua
- Participate in TB/HIV related meeting at National level to provide technical input as needed
- Review and suggest necessary revisions to associated guidelines, protocols, training curricula, training materials and TB/HIV related job aides
- Assist NAP and NTP as well as MOH on development of Viral Load testing strategy that integrated into TB/HIV testing strategy
- Analyze HIV Cascade data especially for viral load data for Indonesia and Jakarta and suggest improvements to reduce barriers in HIV service provision (test-treat-retain) for key populations;
- Develop quarterly TA plans and site visit schedules with PHO and 5 DHOs in Jakarta
- Participate in at least quarterly site QA and QI visits, in collaboration with relevant PHO or DHO officials.
- Participate in any LINKAGES activity as requested
The consultancy will be based in Jakarta. Some travel within Indonesia may be required.
Period of Performance
November 28, 2016 through May 31, 2017 for a total of 90 working days (including 10 days anticipated travel to Papua).
Deliverables/Milestone
- Quarterly TB/HIV clinical mentoring visits to targeted TA-SDI facilities conducted at least 10 targeted health facilities
- Support PHO/DHO on implementation TB/HIV integration services at selected health facilities
- Assist Subdit TB and HIV to Develop strategy and protocol required for GeneXpert testing for presumptive TB among PLHIV
- Provide technical input at TB/HIV district planning exercise meeting in DKI Jakarta and Papua
- Develop with PHO/DHO Fast-track VL testing annual testing target in DKI Jakarta
- Assist the MOH, NAP and NTB to conceptualize an integrated viral load testing policy, strategy and systems using GeneXpert and other VL testing systems
- Analyze current viral load testing data in five hospitals and develop recommendation based on the findings
- Co-facilitation support for development of quarterly PHO and DHO workplans, budgets, and site visit schedules
The consultant will be paid an agreed daily rate for her/his services. Payment will be made upon successful completion and approval of tasks assigned.
Travel logistics and payment for flights/hotels will be paid for directly by FHI. Any incurred travel expenses (such as M&IE and transportation) will be reimbursed upon submission of receipts.
Qualifications
- 10 years related experience and demonstrable knowledge on clinical services and health systems strengthening within Indonesia (either at an NGO or government level)
- More than 10 years experiences working on National HIV Program, and have knowledge on join TB HIV intervention program.
- Completed a Master Degree in Public health, health education, clinical science, nursing, medicine, international development, etc) in implementing technical and organizational capacity building programs.
- Knowledge of community-based HIV/AIDS programming with a special focus on HIV prevention (SBCC, HTC), care, and/ treatment.
- Experience working with key populations including FSW, MSM, and PWID.
- Proven skills in developing organizational capacity of partners in implementing HIV/AIDS programs, including development of training materials, assessments, facilitation, and data collection and analysis.
- Fluency in Bahasa Indonesia and English.
- Strong planning and inter-cultural communication skills required.
- Ability to travel.
- Indonesia resident (Indonesian nationals strongly preferred)
- Experience working on PEPFAR funded projects and knowledge of US government rules and regulations.

Position Title: ART Consultant
Consultant Summary / Responsibilities:
The consultant will work under the direction of the LINKAGES Senior Technical Officer, Clinical Services and in close coordination with the LINKAGES/Indonesia clinical services team to provide senior-level expertise in the area of clinical services capacity building and health systems strengthening. He will represent LINKAGES at senior level meetings, consultations and forums, provide input to strategies, agreements, protocols, guidelines, training curricula and suggest improvements to the district-level HIV capacity building structures and systems. Specific activities may include:
- Support the PHO and other key implementers to ensure test kit availability at Fast Track facilities (including TB/HIV service delivery points).
- Support the coordination of mobile testing strategies at district levels.
- Support a restricted oral fluid test-for triage pilot model in one facility (e.g. Ruang Carlo).
- Advocate for the simplification of pre-testing and work flow procedures in an attempt to shorten the time period between testing, diagnosis and treatment initiation.
- Support performance-based incentive mechanisms implementation and targeted GO! communications to promote the benefits of knowing one’s status including behavioral communications for health care providers.
- Support up to four satellite facilities to initiate ART over the course of the programmatic period, in line with GOI targets.
- Streamline HIV diagnosis and enrollment procedures to facilitate immediate enrollment in HIV care and treatment for those who are diagnosed as HIV positive.
- Explore the extension of operating hours at one targeted puskesmas, in collaboration with the GFATM.
- Support same-day ART initiation pilot in two targeted facilities (Ruang Carlo and Angsamerah).
- Support the roll out of the national clinical mentoring guidelines and the development and/or strengthening of a district-level clinical mentoring system.
- Review patient retention in up to five facilities in an effort to confirm the proportion of patients who have missed appointments, or are LTFU, transfer in/out, or dead. This information will be used to prepare a rigorous patient retention strategy and/or tracking system that systematically brings together community- and facility-based implementers, irrespective of funding source.
